For all those who are attending, please complete the District Liability form AND (new this year) the Physical Examination form. This form must be filled out and signed by your Physician as it will give you the needed clearance to participate in all the exciting activities while at camp.
Please return completed forms to Beth on or before our Orientation Meeting on Sunday, May 2.
